Job summary

Atlantic Health System in Morristown, NJ is seeking a weekday per diem social worker to support patients and families across hospitalization and community care. The role requires availability for at least 2 days per week plus weekend and holiday rotations.

You will assess psychosocial needs, interview patients and families, participate in Multidisciplinary Rounds, advocate for Advanced Care Planning, and coordinate discharge planning with the care team to ensure continuity of care.
