Job summary

OneWell Health Care is seeking a Per Diem Speech-Language Pathologist (SLP) to provide assessment and therapeutic services for individuals with IDD in home health settings across Pennsylvania. This PRN/part-time role requires CCC-SLP, state licensure, and reliable transportation.

You will evaluate, plan, implement and monitor SLP services, including AAC and dysphagia management, while collaborating with families and an interdisciplinary team. Travel to client locations is expected.

Qualifications

• Master’s Degree in Speech-Language Pathology from an accredited program.
• Strong knowledge of speech, language, communication and swallowing disorders.
• Ability to implement AAC systems and individualized treatment plans.
• Excellent documentation, organizational, and communication skills.
• Licensure as a Speech-Language Pathologist in the state.
• CCC-SLP certification from ASHA.

Responsibilities

• Conduct SLP evaluations for speech, language, cognitive-communication, swallowing and AAC.
• Develop, implement, and update individualized treatment plans and goals.
• Provide evidence-based speech-language therapy including AAC, feeding and swallowing interventions.
• Monitor progress and modify treatment plans as needed.
• Collaborate with caregivers, families, and interdisciplinary team members.
• Provide training and consultation on communication, AAC, feeding, and swallowing strategies.
• Maintain timely documentation and ensure regulatory compliance.
• Attend meetings and trainings and participate in quality improvement activities.
