Job summary

OneWell Health Care is seeking a compassionate Speech-Language Pathologist (SLP) to provide personalized therapy to individuals with IDD in home and community-based settings in Pennsylvania. You will assess communication and swallowing needs, develop treatment plans, and collaborate with caregivers and other professionals to improve communication, independence, and quality of life.

Qualifications

• Master's degree in Speech-Language Pathology required.
• Current and valid SLP license in the state of practice.
• CCC-SLP certification from ASHA preferred.
• Strong clinical, communication, documentation and organizational skills.
• Valid driver's license and willingness to travel to home/community sites.
• Experience with IDD, home/community services, AAC or dysphagia preferred.
• English proficiency required; Spanish a plus.

Responsibilities

• Conduct speech, language, cognitive-communication, AAC, and swallowing evaluations.
• Develop and update individualized treatment plans.
• Provide evidence-based speech, communication, AAC, feeding and swallowing therapy.
• Collaborate with families, caregivers and interdisciplinary team members.
• Educate caregivers on communication strategies, AAC use, and safe swallowing practices.
• Complete accurate and timely clinical documentation.
• Participate in required meetings and trainings.

You will assess communication and swallowing needs, develop individualized treatment plans, and collaborate with caregivers and other professionals to help individuals improve their communication, independence, and quality of life.

What We're Looking For:

• Master's degree in Speech-Language Pathology from an accredited program.
• Current and valid Speech-Language Pathologist license in the state of practice.
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA.
• Strong clinical, communication, documentation, and organizational skills.
• Valid driver's license, reliable transportation, and willingness to travel to home and community-based service locations.
• Experience supporting individuals with Intellectual and Developmental Disabilities (IDD), providing home or community-based services, and working with AAC or dysphagia management is preferred.
• English proficiency is required; bilingual proficiency, including Spanish, is a plus.
