Job summary

Elderwood at North Creek is seeking a compassionate PRN Speech-Language Pathologist to provide high-quality therapy in a skilled nursing setting. This per diem role offers flexible hours across weekdays, weekends, and holidays.

The SLP will conduct evaluations, develop plans of care, and collaborate with an interdisciplinary team to support residents’ communication, cognitive, and swallowing needs.

Qualifications

• Current New York State license as a Speech-Language Pathologist.
• CCC-SLP certification preferred.
• Experience in a skilled nursing facility preferred.
• Strong clinical, organizational, and communication skills.
• Ability to work independently and as part of a team.
• Flexibility to work weekdays, weekends, and holidays.

Responsibilities

• Evaluate, treat, and document speech, language, cognitive, and swallowing disorders per physician orders and plan of care.
• Develop and implement individualized treatment plans.
• Maintain accurate, timely documentation per regulatory standards.
• Collaborate with nursing, therapy team, physicians, and families.
• Participate in care planning and interdisciplinary team meetings.
• Screen residents and coordinate audiometric screenings as needed.
• Attend staff meetings and contribute to in-service trainings.
