Job summary

Elderwood at North Creek is seeking a compassionate PRN Speech-Language Pathologist (SLP) to provide therapy to residents in a skilled nursing setting. The per diem schedule offers flexibility to pick up hours based on facility needs, including weekdays, weekends, and holidays.

The SLP will evaluate and treat speech, language, cognitive, and swallowing disorders, document care, and collaborate with an interdisciplinary team to deliver high-quality care.

Qualifications

• Current New York State license as a Speech-Language Pathologist required.
• CCC-SLP certification preferred.
• Experience in a skilled nursing facility preferred.
• Strong clinical, organizational, and communication skills.
• Ability to work independently and as part of a team.
• Flexibility to work as needed based on facility demands.

Responsibilities

• Evaluate, treat, and document speech, language, cognitive, and swallowing disorders in accordance with physician orders and plan of care.
• Develop and implement individualized treatment plans.
• Maintain accurate, timely documentation in compliance with regulatory standards.
• Collaborate with nursing, therapy team members, physicians, and families.
• Participate in care planning and interdisciplinary team meetings.
• Attend staff meetings and required or preferred in-service training.
