Required CT RN license and BLS certification; BSN preferred. Proficiency with electronic patient records and the ability to use internet literature for care planning are essential.

Responsabilidades

• Assesses the patient’s physical and psychosocial condition throughout the care continuum.
• Develops, communicates and documents a plan of care with patient/family and healthcare team.
• Implements and coordinates the plan of care with the healthcare team.
• Evaluates the plan’s effectiveness and readiness for discharge; revises as needed.
• Plans for patient and family education based on learning needs.
