Yale NewHaven Health is seeking a Registered Nurse to deliver direct patient care on a nursing unit, under supervision of the Nurse Manager/Clinical Coordinator. Responsibilities include assessing, planning, implementing, and evaluating care, supervising non-RN staff, and acting as charge nurse as needed.

CT RN license and BLS are required; BSN preferred; strong computer skills with electronic health records are essential.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Valid State of Connecticut Registered Nurse license.
• BLS required; proficient in electronic patient records.
• Experience not specified.

Responsabilités

• Assesses the patient’s physical and psychosocial condition throughout the continuum of care.
• Develops, communicates and documents a plan of care with patient, family and healthcare team.
• Implements and coordinates the plan of care in collaboration with the team.
• Evaluates the effectiveness of the plan of care and readiness for discharge.
• Provides education to patient and family after assessing learning needs.
• Functions as a preceptor to new staff and as charge nurse when needed.
