Qualifications

• Specialized Diploma in nursing from an accredited school.
• Knowledge of patient- and family-centered care and collaboration with each patient and family.
• Cultural awareness and ability to work with diverse backgrounds.
• Effective communication and information-sharing with patients, families, and staff.
• Ability to delegate appropriately within a care team.
• Knowledge of patient assessment and care planning.
• Education of patients and families about condition, treatment, and recovery steps.
• Knowledge of medication administration and post-discharge care planning.
• Discharge planning and arranging follow-up care.
• Documentation of health information in written or electronic form.

Responsibilities

• Collaborate with patients, families, and care partners to identify learning needs and priorities.
• Incorporate evidence-based practice and clinical expertise in nursing care.
• Use information technology to communicate, manage knowledge, and reduce errors.
• Monitor outcomes of care processes and participate in quality improvement initiatives.
• Work within inter-professional teams, fostering open communication and mutual respect.
• Engage in safety and quality improvement committees as appropriate.
• Provide ongoing patient and family education and support.
