Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required
• Knowledge of patient-and family-centered care and ability to collaborate with patients and care partners
• Cultural awareness and ability to work with diverse populations
• Understanding of effective communication and ability to engage patients and families in care decisions
• Knowledge of patient assessment and care planning
• Knowledge of education for patients and care partners
• Knowledge of medication administration and prescription
• Knowledge of discharge planning and post-discharge needs
• Knowledge of health information documentation and accurate medical records

Responsibilities

• Interacts with co-workers, patients, their family/designated care partner(s), and other staff.
• Incorporates evidence-based practice and clinical expertise to deliver optimal healthcare.
• Identifies learning needs of patients and care partners and provides effective education.
