Job summary

Constellation Health Services seeks a compassionate Registered Nurse for per diem work in Kennebunk, ME, covering home health, hospice, assisted living, and skilled nursing facilities. Expect 16–20 hours weekly with weekend/holiday coverage as needed.

You will assess patients, administer medications, coordinate plans of care, and document in our system, collaborating with the nursing supervisor and interdisciplinary team.

Qualifications

• Graduate of an accredited RN program.
• Licensed Registered Nurse in the state of employment.
• 1-2 years of RN experience; hospice care preferred.
• Basic knowledge of iOS operating system.

Responsibilities

• Provide skilled nursing care and med administration in patients' homes and facilities.
• Perform comprehensive assessments and coordinate the plan of care with the team and family.
• Observe, evaluate and report changes to the nursing supervisor and physician.
• Participate in weekly Interdisciplinary Team Meetings and document accordingly.
• Maintain and submit required clinical documentation in a timely manner.
• Provide input for hospice aide evaluation as needed.
• Weekend and holiday coverage as required.
