Constellation Health Services is seeking a PRN Licensed Practical Nurse to provide end-of-life care in patients' homes in Portland, Maine. You will work evenings and weekends under the guidance of an RN, performing assessments, implementing care plans, and ensuring compassionate, high-quality outcomes.

Responsibilities include patient education, care coordination with RNs and social workers, and meticulous documentation.

Qualifications

• Graduate of an accredited practical nurse program.
• LPN licensed in the state of employment.
• Hospice experience preferred.
• Background in chart reviews is a plus.
• Wound care experience preferred.
• Basic knowledge of iOS operating systems.

Responsibilities

• Deliver nursing care in accordance with the State Nurse Practice Act with a focus on end-of-life care.
• Implement nursing practices based on assessments and care plans.
• Maintain accurate and timely documentation of patient services.
• Provide patient and family education and support.
• Collaborate with interdisciplinary teams (RNs, hospice aides, social workers, etc.).
• Participate in Hospice IDT meetings to ensure coordinated care.
• Manage pain and symptoms, evaluating response to treatments.
