Job summary

Americare is seeking a Per Diem RN Intake Coordinator to manage referrals and process appropriate home care services. This role is remote (US) and part-time, with a focus on maintaining clear communication with referral sources and Americare departments to ensure seamless patient intake.

The ideal candidate holds an NYS RN license, is experienced with Allscripts and Home Care Home Base, and demonstrates excellent communication, high organization, and attention to detail.

Qualifications

• Registered Nurse responsible for intake referrals and processing appropriate home care services.
• Maintains open lines of communication with referral sources and Americare departments.
• Provides positive customer service to promote Americare.

Responsibilities

• Take referrals and determine eligibility for home care services.
