Job summary

The nurse will assess physiologic and psychosocial health, develop and implement individualized care plans, administer medications, and participate in discharge planning and family education. Strong documentation and teamwork are essential.

Qualifications

• Graduate from an accredited School of Nursing. Bachelors Degree in Nursing, preferred. Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010 and obtain a BSN degree within five (5) years of employment date.
• Current license to practice as a Registered Professional Nurse in New York State.
• BCLS, ACLS, preferred.
• 2+ years of experience in ED, telemetry, medsurg preferred.

Responsibilities

• Assesses the patients physiologic health status.
• Assesses patients psychosocial health status.
• Identifies patient outcome and develops criteria for measurement.
• Formulates the plan of care and prioritizes to achieve outcomes.
• Implements and documents the plan of care, ensuring safety and quality.
• Evaluates care provided and monitors outcomes for improvement.
• Collaborates with physicians and care team to coordinate treatment.
• Performs skin care and wound management.
• Administers medications correctly and safely.
• Participates in patient and family education and discharge planning.
