Qualifications

• Must possess a current RN license in state of practice and BLS certification.
• Minimum one year of acute care hospital experience in the specialty area.
• Arizona fingerprint clearance card for certain Banner facilities and post-offer background checks.
• Bachelor's degree preferred; professional certification may be advantageous.

Responsibilities

• Assess patient needs across physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
• Formulate a plan of care with patient/family and health team input; set realistic goals.
• Implement the plan of care through direct patient care and supervision of staff.
• Evaluate patient progress and adjust the care plan as needed.
• Document assessments, plans, interventions and outcomes in the patient record.
• Collaborate with multidisciplinary team to ensure quality outcomes and continuity of care.
