Job summary

Jefferson Health in Lansdale, PA seeks a professional nurse to deliver patient- and family-centered care using the nursing process. You will develop individualized plans of care with patients and designated care partners, aligning with their cultural beliefs and values while ensuring safety and quality.

Responsibilities include collaborating with care teams, educating patients and families, utilizing health information systems, and participating in quality improvement efforts to advance outcomes

Qualifications

• Specialized Diploma Graduate of an accredited school of nursing required.
• Cultural Awareness and respect for diverse backgrounds in care planning.
• Effective communication with patients, families and staff throughout care.
• Knowledge of discharge planning processes and post-discharge needs.
• Documentation of health information and patient data in electronic or written form.

Responsibilities

• Interacts with co-workers, patients, their family/designated care partner(s), and other staff consistent with the values of Jefferson Health.
• Partners with patients and families in identifying learning needs and priorities and provides education.
