Job summary

Baystate Wing Hospital in Palmer, MA is seeking an RN Case Manager for per diem coverage. The role coordinates clinical care, quality outcomes, and post-acute transitions with an interdisciplinary team to develop complex discharge plans.

Qualifications

• BSN preferred.
• 3-5 years clinical nursing experience.
• Hospital case management/care coordination experience preferred.

Responsibilities

• Facilitates patient care plan as per plan of care.
• Leads daily patient care huddles to direct timely progress.
• Oversees resource use, patient satisfaction, and quality of care with input from health care professionals.
• Assesses admissions appropriateness using evidence-based criteria.
• Collaborates with payers and physicians to ensure safe readmissions and transitions.
