Job summary

Qualifications

• Minimum two (2) years clinical experience as an RN in an acute care setting.
• ECC Antelope Valley: One (1) year of Utilization Management experience, to include Discharge Planning.

Responsibilities

• Develops an individual care plan with the healthcare team based on assessment and tests.
• Monitors patient progress and modifies the plan with multidisciplinary input.
• Recommends levels of care and referrals for therapy/rehabilitation when indicated.
• Educates patients and families on self-management and care pathways.
• Coordinates transfers and discharge planning to ensure safe transitions.
