Job summary

UCI Health is seeking an experienced Case Manager to lead high-level assessments and care planning for patients and families. You will coordinate interdisciplinary rounds, champion patient-centered self-care, and ensure accurate documentation across the care continuum.

Ideal candidates have 5+ years of clinical experience with at least 1 year in case management, hold a BSN, and demonstrate familiarity with regulatory requirements (Joint Commission, CMS, CCS).

Qualifications

• Minimum 5 years of clinical experience and 1 year of Case Management experience.
• Current licensed RN, Social Worker (MSW or LCSW), or Licensed Healthcare Therapist.
• Familiarity with Joint Commission, CMS, CDPH and CCS requirements.
• Excellent written and verbal communication skills in English.
• Computer literacy and proficiency.
• All external candidates must have a BSN.
• Ability to manage multiple concurrent projects and maintain workload pace.
• Ability to lead interdisciplinary rounds and collaborate across teams.

Responsibilities

• Assess and reassess patient/family needs to set goals and plan care.
• Coordinate with interdisciplinary care teams to ensure safe, high-quality care transitions.
• Document clinical information clearly in the medical record and advocate for patient self-determination.
