Job summary

UCI Health is seeking an experienced RN Case Manager to lead high‑level assessments, care planning, and coordination across interdisciplinary teams. You will advocate for patient self‑determination and ensure safe, cost‑effective transitions of care.

You should have at least 5 years of clinical experience with 1 year in Case Management or be a BSN-qualified internal candidate. Strong English communication and documentation skills are essential.

Qualifications

• Must have 5+ years of clinical experience and 1 year in Case Management or BSN-qualified UCI employee meeting clinical requirements
• Current license as RN, Social Worker (MSW/LCSW) or Licensed Healthcare Therapist allowing independent assessments
• Bachelor of Science in Nursing required for external candidates
• Demonstrated ability to manage multiple concurrent projects and lead interdisciplinary rounds
• Proficient in written and verbal English communication

Responsibilities

• Conduct high level assessments and reassessments to set priorities and plan patient care
• Coordinate care with interdisciplinary teams to ensure safe, quality and cost-effective outcomes
• Advocate for patient self-determination and culturally competent care
• Ensure medical records reflect patient illness severity and care transitions
