Job summary

Chapters Health System is seeking an Admission RN to assess patients at their residence or in hospitals and to initiate care within our Care Navigation services. The role acts as an ambassador for the organization and ensures a seamless transition to ongoing care.

The ideal candidate holds an RN license in the hiring state, has a BLS certification, and at least 1 year of RN experience (hospice or hospital preferred).

Qualifications

• RN license active in the state of employment.
• BLS certification for healthcare professionals from the American Heart Association or Red Cross.
• Minimum of 1 year of RN experience; hospice or hospital experience preferred.
• Strong clinical assessment skills; ED/ICU preferred.
• Experience with an EMR/EHR system.
• Ability to communicate in English (verbal and written); bilingual in Spanish preferred.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Explain services to potential patients and families with empathy and clarity.
• Identify admission objections or barriers and address them.
• Educate patients/families on admission requirements and immediate needs.
• Complete informed consents and initial assessments per policy.
• Provide information to physicians and initiate Plan of Care for immediate needs.
• Initiate skilled nursing interventions to prevent complications and maximize comfort.
• Obtain physician orders and complete documentation per policy.
• Represent the Company at assigned facilities and coordinate referrals across service lines.
• Maintain timely communication with Care Navigation for follow-up.
