Job summary

Pathways Healthcare is a leading home health and hospice organization focusing on patient-centered care. We offer an AI-assisted documentation program to save clinicians 5–10 hours weekly, while clinicians retain clinical judgment at the center of every note.

Role: Registered Nurse - Home Health with main office in Lawrence, MA, serving Lawrence, Methuen, Andover, North Andover and Haverhill. We value bedside manner, communication, legal knowledge, and commitment to geriatrics.

Qualifications

• Active nursing license in the state where the position is based.
• Preferably 2+ years of nursing experience with a geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust plans of care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan.
