Pathways Healthcare in Westwood, MA is seeking a Registered Nurse - Home Health to deliver patient-centered care in the home setting across Quincy, Braintree, Milton and surrounding areas. You will assess needs, administer medications, and develop plans of care in collaboration with physicians and NPs.

This role requires an active RN license in Massachusetts, 2+ years geriatrics experience, a nursing degree, strong communication, and valid driver's license with access to a reliable vehicle.

Qualifications

• Must hold an active Nursing license in Massachusetts.
• Preferably 2+ years of nursing experience, geriatrics focus.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable vehicle.

Responsabilités

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform comprehensive physical assessments and histories.
• Develop and revise care plans based on patient status.
• Collaborate with physicians, NPs, and teams to create robust care plans.
• Address inquiries from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them as part of the plan.

Job Description:

• Role: Registered Nurse - Home Health
• Main Office location: Westwood, MA
• Service Area: Quincy, Braintree, Milton and Surrounding Areas
• Schedule: Monday - Friday (8:30-5:00pm)
• Job Type: Full Time

• Initiate appropriate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Responsively address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
