Job summary

Pathways Healthcare, Llc is seeking an experienced Registered Nurse to join our physician- and nurse practitioner-led home health and hospice team in Concord, MA. You will assess patients, coordinate in-home care, and support families in recovery at home.

You will administer medications, develop individualized plans of care, collaborate with physicians, and mentor home health aides. A geriatrics focus and strong communication are essential, with a valid MA RN license and BLS CPR.

Qualifications

• Must hold an active RN license in the state MA.
• Preferably 2+ years of nursing experience with geriatrics.
• Nursing degree from an accredited institution.
• Excellent bedside manners and exceptional communication skills.
• Working knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and treatments as prescribed by physicians.
• Conduct initial assessments of patients and families to determine home care needs.
• Perform thorough physical assessments and gather comprehensive medical histories.
• Develop and revise individualized plans of care based on patient status and evolving needs.
• Collaborate with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support to patients and caregivers before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and proactively implement them as part of the care plan development.
