Job summary

Willow Creek Health Care Center is seeking a compassionate Per Diem Physical Therapist Assistant to join our rehabilitation team. Under supervision of a licensed Physical Therapist, you will provide skilled treatment to improve residents' strength, balance, mobility, safety and independence.

The role involves performing therapeutic exercises, gait training, transfers, and mobility activities, with documentation and compliance to policy.

Qualifications

• Associate degree from an accredited Physical Therapist Assistant program required.
• Current California Physical Therapist Assistant license in good standing required.
• Current CPR certification when required by licensure or facility policy.
• Geriatric, skilled nursing or post-acute rehab experience preferred.

Responsibilities

• Provide PTA services under the supervising PT's plan of care.
• Administer therapeutic exercises, gait and mobility training, and transfer coaching.
• Monitor patient responses; report progress/concerns to the supervising PT.
