Job summary

Willow Creek Health Care Center is seeking a compassionate Per Diem Physical Therapist Assistant (PTA) to join our rehabilitation team. Under the supervision of a licensed Physical Therapist, you will provide skilled treatment to improve residents’ strength, balance, mobility, safety, and independence.

This role offers flexible per-diem scheduling, collaborative care with a dedicated rehabilitation team, and opportunities to educate residents and families on home exercise and fall-prevention

Qualifications

• Associate degree from an accredited Physical Therapist Assistant program required.
• Current California Physical Therapist Assistant license in good standing required.
• Current CPR certification when required by licensure or facility policy.

Responsibilities

• Provide PTA treatment under supervision per plan of care.
• Carry out therapeutic exercises, gait and mobility training.
• Monitor patient responses and communicate progress to PT.
• Complete accurate treatment documentation.
