Tender Touch Rehab Services is seeking a licensed Physical Therapist Assistant for per diem work in SNF and long-term care settings. You will provide PT treatments under a licensed PT, and help identify suitable patients for therapy while coordinating evaluations and referrals.

The role emphasizes collaborative care, documentation compliance, and timely processing of physician orders, with a focus on high-quality patient outcomes.

Qualifications

• Must be licensed as a Physical Therapist Assistant in the state of practice.
• Associate’s degree from an APTA-accredited PTA program.
• Strong communication and organizational skills.

Responsibilities

• Provide physical therapy treatments under the supervision of a licensed Physical Therapist.
• Assist in identifying suitable patients for therapy and coordinate with the PT for evaluation and referral.
• Collaborate with team to ensure quality outcomes.
• Maintain documentation to meet regulatory requirements.
• Ensure all physician orders are current and on file.
