Qualifications

• Graduate of an APTA accredited program or equivalent.
• Current Physical Therapist License in good standing.
• Proficient documentation and communication skills.

Responsibilities

• Participates in outcome-driven therapy sessions and plans.
• Delivers high-quality care in a safe environment.
• Completes accurate progress notes and discharge summaries.
• Communicates with residents, families, and care teams.
• Executes treatment plans and assists with DME considerations.
• Maintains compliance with departmental policies and procedures.
