California Rehabilitation and Sport Therapy in Morgan Hill, CA is seeking a Per Diem Physical Therapist to join our outpatient team and support flexible scheduling. You will provide expert rehabilitation care, develop individualized plans, and help patients regain mobility and reduce pain.

Formación

• State licensure as a Physical Therapist is required.
• DPT or MS in Physical Therapy from an accredited program.
• CPR certification or state-required certifications.
• Experience in orthopedic, neurological, or geriatric PT preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and within a team.
• Proficient in documentation and record-keeping.

Responsabilidades

• Conduct comprehensive assessments to determine functional abilities, limitations, and goals.
• Develop personalized treatment plans based on assessments and goals.
• Implement exercises, manual therapy, and other interventions to improve mobility and function.
• Maintain accurate documentation of patient progress and outcomes.
• Monitor progress and adjust treatment plans as needed.
• Educate patients and families about therapy procedures and home exercises.
• Collaborate with physicians, nurses and other healthcare professionals.
• Delegate and utilize PTA when applicable.
• Stay updated on advancements and integrate them into practice.
