California Rehabilitation and Sport Therapy is seeking a Per Diem Physical Therapist to support a flexible schedule in Santa Clara, CA. You will provide high-quality rehab services, assess patients, and tailor treatment plans to improve mobility and reduce pain.

Formación

• Must hold valid state licensure as a Physical Therapist.
• Doctorate or Master\'s degree in Physical Therapy from an accredited program.
• CPR certification and other state-required certifications.
• Strong clinical skills in orthopedic, neurological, or geriatric PT (preferred).
• Excellent communication and teamwork abilities.
• Proficient computer skills for documentation.

Responsabilidades

• Conduct comprehensive assessments to determine patients\' functional abilities, limitations, and goals.
• Develop personalized treatment plans based on assessments and goals, utilizing various therapeutic techniques and modalities.
• Implement and administer therapeutic exercises, manual therapy, and other interventions to improve mobility, strength, flexibility, and function.
• Maintain accurate and organized documentation of patient progress and treatment outcomes.
• Monitor and document patients\' progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Educate patients and their families about therapy procedures, home exercises, and injury prevention techniques.
• Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive patient care.
• Stay updated on advancements in physical therapy techniques, technologies, and best practices, integrating them into clinical practice as appropriate.
• Meet the needs of having patients execute Plan of Care and (but not limited) following up with patients if noncompliance occurs.
• Be a team player to utilize nonclinical staff to support needs of the caseload.
• Delegate and use a Physical Therapy Assistant (PTA) when applicable.
• Participate in helping to drive a collaborative culture based on our service vision.
• Adapt to change in patient schedules and daily patterns.
• Adhere to ethical and professional standards, ensuring compliance with regulatory requirements and company policies.
• Perform comprehensive patient assessments to determine therapy needs and establish treatment goals.
