California Rehabilitation and Sport Therapy in Morgan Hill, CA is seeking a Per Diem Physical Therapist to join a collaborative clinical team. The role offers flexible weekday and Saturday shifts, enabling clinicians to tailor hours around other positions while delivering high-quality patient care.

The position emphasizes comprehensive assessments, personalized treatment plans, and evidence-based therapies, with a focus on improving mobility, reducing pain, and enhancing patient outcomes.

Qualifications

• Valid state licensure as a Physical Therapist.
• Doctorate or Master's degree in Physical Therapy from an accredited program.
• CPR certification and any other relevant certifications as required by state regulations.
• Strong clinical skills with experience in orthopedic, neurological, or geriatric physical therapy (preferred).
• Excellent communication and interpersonal skills.
• Ability to work effectively both independently and collaboratively within a team environment.
• Proficient computer skills for documentation and record-keeping purposes.

Responsibilities

• Conduct comprehensive assessments to determine patients' functional abilities, limitations, and goals.
• Develop personalized treatment plans based on assessments and goals, utilizing various therapeutic techniques and modalities.
• Implement and administer therapeutic exercises, manual therapy, and other interventions to improve patients' mobility, strength, flexibility, and function based on each individual patient’s need.
• Maintain accurate and organized documentation of patient progress and treatment outcomes.
• Monitor and document patients' progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Educate patients and their families about therapy procedures, home exercises, and injury prevention techniques.
• Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive patient care.
• Delegate and use a Physical Therapy Assistant (PTA) when applicable.
• Adapt to change in patient schedules and daily patterns.
• Adhere to ethical and professional standards, ensuring compliance with regulatory requirements and company policies.
