California Rehabilitation and Sports Therapy is seeking a Per Diem Physical Therapist in San Jose, CA. This role offers flexible scheduling with shifts including Saturdays, allowing you to tailor your work around other commitments while delivering high-quality outpatient care.

You will perform assessments, design personalized treatment plans, and collaborate with a supportive clinical team to improve mobility and reduce pain.

Formación

• Valid state licensure as a Physical Therapist.
• Doctorate or Master’s degree in Physical Therapy from an accredited program.
• CPR certification and any other state-required credentials.
• Strong clinical skills with experience in orthopedic, neurological, or geriatric PT (preferred).
• Excellent communication and interpersonal skills.
• Ability to work independently and collaboratively within a team.
• Proficient computer skills for documentation and record-keeping purposes.

Responsabilidades

• Conduct comprehensive assessments to determine patients' functional abilities, limitations, and goals.
• Develop personalized treatment plans based on assessments and goals, utilizing various therapeutic techniques and modalities.
• Implement and administer therapeutic exercises, manual therapy, and other interventions to improve mobility, strength, flexibility, and function.
• Maintain accurate and organized documentation of patient progress and treatment outcomes.
• Monitor and document patients' progress, adjusting treatment plans as necessary to achieve optimal outcomes.
• Educate patients and families about therapy procedures, home exercises, and injury prevention techniques.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Maintain timely documentation of patient evaluations, progress notes, and discharge summaries.
