Job summary

California Rehabilitation & Sports Therapy is seeking a Physical Therapist for a per diem role in San Jose, CA. You will provide high-quality rehabilitation services, assess patients, and create individualized treatment plans while coordinating care with a multidisciplinary team.

The position offers flexible scheduling with shifts throughout the week, including Saturdays, and a supportive clinical environment that values patient outcomes and professional growth.

Qualifications

• Valid state licensure as a Physical Therapist.
• Doctorate or Master's degree in Physical Therapy from an accredited program.
• CPR certification and any other relevant certifications as required by state regulations.
• Experience in orthopedic, neurological, or geriatric PT preferred.

Responsibilities

• Conduct comprehensive assessments to determine patients' functional abilities, goals, and limitations.
• Develop and implement personalized treatment plans.
• Administer therapeutic exercises and modalities to improve mobility and function.
• Maintain accurate documentation of progress and outcomes.
• Educate patients and families on therapy procedures and home programs.
• Collaborate with physicians, nurses and other clinicians to ensure integrated care.
• Stay up-to-date with PT advances and apply best practices.
