Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. The role includes managing acute changes in condition, advanced care planning, and chronic care management within an interdisciplinary team.

Applicants must hold an unrestricted Kentucky NP license, board certification, and DEA licensure. Experience with EMR systems and LTC settings is preferred.

Qualifications

• Unrestricted NP license in Kentucky (state license).
• Board certification as NP.
• DEA licensure for prescribing controlled substances.
• Experience with adult/family/geriatric populations.
• Knowledge of long-term care standards and practice.
• Ability to work in an interdisciplinary team.
• Experience with EMR systems.

Responsibilities

• Provide primary and preventative care for long-term care residents in skilled facilities.
• Manage residents with acute changes in condition.
• Conduct advanced care planning and chronic care management.
• Prescribe medications in accordance with Kentucky law.
• Order and follow up on treatments and diagnostic tests.
• Communicate changes in condition with nursing home staff.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.




• Manage Residents with acute change in condition.




• Advanced care planning and chronic care management.




• Prescribe appropriate medications, including controlled substances in accordance with KY Law.




• Order, review and/or follow-up with treatments and diagnostic testing.




• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.




• and/or the care plan.




• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.




• Board Certification.




• DEA licensure.




• Clinical background in adult, family or geriatric specialties.




• Experience working in a long-term care setting.




• Ability to work with an EMR.




• Ability to manage patients with advanced illness and multiple chronic conditions.




• Ability to work within an interdisciplinary care team to coordinate patient care.
