Job summary

CareMed is seeking a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities in Kentucky.

You will manage residents with acute changes in condition, perform advanced care planning, and prescribe medications in accordance with KY law.

Collaborating with an interdisciplinary team and a primary care physician, you will review treatments, coordinate care plans, and communicate effectively with facility staff using an EMR system.

Qualifications

• Current, unrestricted NP license in KY.
• Board certification as NP.
• DEA licensure.
• Experience in long-term care settings.
• Proficiency with EMR systems.
• Ability to work within an interdisciplinary care team.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.

• Manage Residents with acute change in condition.

• Advanced care planning and chronic care management.

• Prescribe appropriate medications, including controlled substances in accordance with KY Law.

• Order, review and/or follow-up with treatments and diagnostic testing.

• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

• and/or the care plan.

• Collaborate with primary care physician.
  

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of KY

• Board Certification.

• DEA licensure.

• Clinical background in adult, family or geriatric specialties.

• Experience working in a long-term care setting.

• Ability to work with an EMR.

• Ability to manage patients with advanced illness and multiple chronic conditions.

• Ability to work within an interdisciplinary care team to coordinate patient care.

• Psych experience a plus
