CareMed Health Solutions in Kentucky seeks a Nurse Practitioner to provide primary and preventive care for residents in skilled nursing facilities. You will manage acute changes in condition, engage in advanced care planning, and coordinate with the interdisciplinary team to optimize chronic disease management.

Applicants must hold a KY NP license, board certification, and DEA registration, with willingness to work in a long-term care setting and operate within an EMR system.

Formación

• Current, unrestricted NP license in KY.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care settings.
• Ability to use an EMR.

Responsabilidades

• Provide primary and preventive care for long-term care residents in skilled nursing facilities.
• Manage residents with acute changes in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per KY law.
• Order, review and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff about changes in condition and care plan.
• Collaborate with the primary care physician.

NP ROLE AND RESPONSIBILITIES

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP Qualifications And Education Requirements

• Current, unrestricted NP license in the state of KY
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus
