CareMed Health Solutions is seeking Nurse Practitioners to provide medical services to residents in long-term care facilities. You will deliver primary and preventive care, address acute changes in condition, and participate in care planning with an interdisciplinary team.

This role requires Kentucky NP license, board certification, DEA, and experience in geriatric or adult care. You will use EMR systems, coordinate with facility staff, and support residents’ comfort and quality of life.

• Current, unrestricted NP license in Kentucky.
• Board Certification.

• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care settings.
• Proficient with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.


• Manage Residents with acute change in condition.


• Advanced care planning and chronic care management.


• Prescribe appropriate medications, including controlled substances in accordance with KY Law.


• Order, review and/or follow-up with treatments and diagnostic testing.


• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.


• and/or the care plan.


• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.


• Board Certification.


• DEA licensure.


• Clinical background in adult, family or geriatric specialties.


• Experience working in a long-term care setting.


• Ability to work with an EMR.


• Ability to manage patients with advanced illness and multiple chronic conditions.
