CareMed is seeking a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities. You will manage acute changes, engage in advanced care planning, and collaborate with nursing home staff and primary care physicians to coordinate patient care.

You will prescribe medications under Kentucky law, utilize an EMR, and support residents with multiple chronic conditions within an interdisciplinary team.

Qualifikationen

• Current, unrestricted NP license in Kentucky.
• Board Certification as Nurse Practitioner.
• DEA licensure to prescribe controlled substances.
• Clinical background in adult, family or geriatric care.
• Experience in long-term care settings.
• Ability to work with an EMR and coordinate with a care team.
• Capability to manage patients with multiple chronic conditions.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES:

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with KY Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

• Current, unrestricted NP license in the state of Kentucky.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
