Job summary

The Oaks at Denville, located in Denville, NJ, is seeking an experienced Per Diem Physical Therapist to join our team. The therapist will assess, plan, and administer rehabilitative programs to improve mobility and reduce pain.

Responsibilities include initial exams, designing personalized treatment plans, documenting progress, and collaborating with the care team. A current NJ PT license and a background in rehab settings are preferred; pay is $59 per hour.

Qualifications

• Current NJ Physical Therapist State License.
• Graduate of an accredited therapy program.
• Per Diem availability preferred.
• Experience in SAR/SNF/LTC settings preferred.

Responsibilities

• Perform and document initial exams to identify problems and diagnosis.
• Plan and carry out individualized rehabilitation programs.
• Record prognosis, treatment, and progress in patient records.
• Identify goals and plans for reevaluation.
• Administer manual exercises and modalities to relieve pain and improve function.
• Test strength, motor development, and functional capacity; record data.
• Instruct patients and families for at-home treatment.
• Coordinate with physicians and care team.
• Review referrals and medical records to determine treatment.
