Job summary

Village Point is seeking a Per-Diem Physical Therapist to assess, plan, and administer rehabilitative programs that improve mobility and restore function in residents of a Springpoint Senior Living Community in Monroe Township, NJ.

The role requires an active NJ Physical Therapist license and graduation from an accredited program. You will perform initial exams, document progress, and educate patients and families. Competitive hourly pay and benefits are included.

Qualifications

• High school diploma or equivalent required.
• Graduate of an accredited therapy program.
• Current NJ Physical Therapist State License.
• Experience with rehabilitative programs and patient mobility improvements.

Responsibilities

• Perform initial exam and evaluate data to identify problems and determine a diagnosis prior to intervention.
• Plan and carry out individualized physical therapy programs to improve mobility and reduce pain.
• Record prognosis, treatment, response, and progress in patient records or computer system.
• Identify goals, progress, and plans for reevaluation with patients and families.
• Administer manual therapies, massage, or traction to relieve pain and improve strength.
• Test strength, range of motion, sensation, and functional capacity; record data.
• Instruct patients and families in home treatment procedures.
• Coordinate with physicians and other team members to plan interventions.
• Review physician referrals and patient medical records to determine required therapy.
