Job summary

McKinley Park Care Center in Sacramento, CA is seeking a Per-Diem Physical Therapist to evaluate residents, develop individualized treatment plans, and provide therapy to improve mobility and independence. You will collaborate with physicians, nursing staff, families, and the rehab team to optimize outcomes.

The PT will participate in staff education, quality improvement initiatives, and regulatory compliance while benefiting from a flexible per-diem schedule and modern rehabilitation resources.

Qualifications

• Evaluate residents' physical therapy needs, functional abilities, and rehabilitation potential.
• Develop, implement, and revise individualized treatment plans based on physician orders and clinical findings.
• Provide evidence-based interventions to improve mobility, strength, balance, endurance, coordination, and functional independence.
• Establish realistic goals and monitor progress toward achievement.
• Educate residents and families on treatment plans, home exercise programs, fall prevention, and safety techniques.
• Recommend appropriate adaptive equipment and assistive devices.
• Communicate resident progress and treatment recommendations to physicians, nursing staff, families, and interdisciplinary team members.
• Participate in care plan meetings, case conferences, rehabilitation meetings, and discharge planning discussions.
• Collaborate with rehabilitation staff to maximize resident outcomes.
• Support restorative nursing and rehabilitation programs.
• Provide supervision and direction to PTAs, rehabilitation aides, and students.
