Responsibilities

• Evaluate residents' physical therapy needs, functional abilities, and rehabilitation potential.
• Develop, implement, and revise individualized treatment plans based on physician orders and clinical findings.
• Provide evidence-based interventions to improve mobility, strength, balance, endurance, coordination, and functional independence.
• Educate residents and families on treatment plans, home exercise programs, fall prevention, and safety techniques.
• Collaborate with physicians, nursing staff, families, and rehabilitation team members to achieve optimal outcomes.
