Job summary

Select Medical Rehabilitation Hospital at Lutheran Hospital in Fort Wayne, IN is seeking a Physical Therapist for per diem weekday and weekend coverage. The position offers $55.00 per hour with a $5,000 sign-on bonus and state licensure required.

You will deliver inpatient rehabilitation therapy, conduct patient assessments, develop treatment plans, supervise PTAs and students, and document according to department policies while contributing to quality improvement and discharge planning.

Qualifications

• State licensure as a Physical Therapist (PT) is required.
• Certified BLS (or obtain within 90 days of employment).

Responsibilities

• Perform initial and ongoing systematic patient assessments.
• Develop and implement individualized treatment plans.
• Supervise Physical Therapy Assistants (PTAs) and PT/PTA students.
• Complete accurate documentation per department policies and procedures.
• Participate in discharge planning and patient/family education.
