Job summary

Fort Wayne Health Campus in Fort Wayne, Indiana, is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. You will evaluate and treat residents with orthopedic, neurological, and cardiovascular conditions, and work with residents, families, and other professionals to design effective plans.

Qualifications

• Physical Therapist (PT) degree from an accredited institution.
• Current licensure in the state of Indiana.

Responsibilities

• Evaluate and treat residents with orthopedic, neurological, and cardiovascular disorders.
• Develop and implement individualized treatment plans with residents, families, and healthcare professionals.
• Conduct PT assessments and progress reports to measure effectiveness and adjust treatment.
• Utilize therapy modalities including exercise prescription, manual therapy, and modalities.
• Collaborate with physicians, occupational therapists, and speech-language pathologists for comprehensive care.
• Maintain accurate documentation of resident care and outcomes.
