Fort Wayne Health Campus in Fort Wayne, Indiana, is seeking a dedicated Physical Therapist (PT) to join our rehabilitation team. You will evaluate and treat residents with orthopedic, neurological, and cardiovascular conditions, and work with residents, families, and other professionals to design effective plans.

• Utilize therapy modalities including exercise prescription, manual therapy, and modalities.
• Collaborate with physicians, occupational therapists, and speech-language pathologists for comprehensive care.
• Maintain accurate documentation of resident care and outcomes.

Our Physical Therapy Department is a vital part of our care team, working closely with residents, families, and other healthcare professionals to achieve optimal outcomes.

• Evaluate and treat residents with a variety of physical disabilities and conditions, including but not limited to, orthopedic, neurological, and cardiovascular disorders
• Develop and implement individualized treatment plans in collaboration with residents, families, and other healthcare professionals
• Conduct physical therapy assessments and progress reports to measure treatment effectiveness and make necessary adjustments
• Utilize a variety of therapy modalities, including but not limited to, exercise prescription, manual therapy, and modalities
• Collaborate with other healthcare professionals, including but not limited to, physicians, occupational therapists, and speech-language pathologists, to ensure comprehensive care
• Maintain accurate and thorough documentation of resident care and treatment outcomes
