Job summary

Bridgeport Hospital, part of Yale New Haven Health, is seeking a dedicated hospital pharmacist to join our patient-centered care team. You will ensure safe medication use, resolve distribution issues, and collaborate with physicians and nurses to optimize patient outcomes. The ideal candidate holds a Pharm.D.

or B.S. in Pharmacy, Connecticut license (or eligible for reciprocity), and at least two years of acute care experience.

Qualifications

• Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D.
• Residency in hospital pharmacy desirable but not required.
• Two years experience in an acute care hospital pharmacy is preferred.
• Licensed to practice pharmacy in Connecticut (or eligible for reciprocation).
• Knowledge of pharmacology, therapeutics, and formulary management.
• Excellent communication skills and ability to work with others.

Responsibilities

• Resolves drug distribution problems assuring accurate and timely dose delivery to all patients.
• Interprets clinical and pharmacokinetic lab data to formulate medication monitoring plans.
• Problem identification and resolution; communicates and documents findings and follow-up.
• Provides drug information to nurses, MDs, and other health care professionals.
• Participates in educational forums within the department.
• Documents medication use variances (MUVs) and adverse drug reactions (ADRs).
• Demonstrates high reliability organization principles and patient safety focus.

Responsibilities

• 1.Resolves drug distribution problems assuring accurate and timely dose delivery to all patients
  • 1.1Responds to telephone requests for medication in appropriate fashion accommodating callers request or routing call to appropriate person.
• 2.Interprets Clinical and Pharmacokinetic Lab Data in order to formulate medication monitoring plans.
  • 2.1Demonstrates knowledge of basic pharmacokinetic principles.
• 3.Problem Identification & Resolution
  • 3.1Communicates and documents problem detection, relevant findings, recommendations, and necessary follow-up.
• 4.Provides drug information to nurses, MDs, and other health care professionals
  • 4.1Serves as primary source of drug information to physicians, nurses, and other health care professionals
• 5.Educational Activities
  • 5.1Participates in and contributes to educational forums within the department (written or verbal).

Qualifications

EDUCATION

Graduation from an accredited College of Pharmacy with a B.S. or Pharm.D. degree. Completion of a residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is desirable but not required.

EXPERIENCE

Two years experience in an acute care hospital pharmacy is preferred for this position.

LICENSURE

Licensed to practice pharmacy in Connecticut (or eligible for reciprocation)

SPECIAL SKILLS

This individual should demonstrate knowledge of pharmacology and therapeutics, and understand pharmacy systems of drug distribution, and formulary management. Excellent communication skills and the ability to work with others are required.
