Job summary

Qualifications

• High School diploma or GED.
• CNA or HHA certification required.
• Experience with older adults or frail populations preferred.

Responsibilities

• Assist participants with personal care and daily activities.
• Accompany participants to appointments and assist transportation.
• Lead group activities and contribute to activities program.
• Document care and report to care team.
