Qualifications

Responsibilities

• Recognize and respond to patient and family needs.
• Communicate changes in patient condition to nursing team.
• Follow Safe Patient Handling and no-manual-lift practices.
• Participate in Quality Improvement programs.
• Assist with personal care per care plans.
• Document care in the patient’s EMR.
• Report accidents, incidents, and abuse immediately.
• Monitor skin integrity and patient safety.
• Attend care planning meetings as needed.
• Obtain vital signs and basic procedures as directed.
• Maintain infection control practices.
• Maintain patient confidentiality per HIPAA.
