Job summary

AdvisaCare is seeking a dedicated Hospice Social Worker to guide patients and families through the hospice journey with compassion and advocacy. The role emphasizes psychosocial assessment, care planning, and crisis counseling within a support-focused care environment.

Qualified candidates hold an MSW (CSWE-accredited) and an active LMSW/LLMSW license in Michigan, with experience in home health or hospice settings preferred and a valid driver's license.

Qualifications

• MSW degree from CSWE-accredited program
• Active LMSW/LLMSW in Michigan or eligible to practice in US
• Experience with patients and families in hospice/home health preferred

Responsibilities

• Perform psychosocial, emotional, spiritual, and bereavement assessments and assist in care plan development
• Provide care under physician supervision and participate in IDT conferences
• Document visits, contacts, and referrals in patient records
• Conduct reassessments and provide counseling as needed
• Provide crisis intervention and individual/family counseling
• Advocate for patient and family and coordinate with community services
• Offer counseling on death and dying for patients and families
• Participate in Bereavement Team activities
• Adhere to hospice policies

In this role, you will help guide patients and families through the hospice journey with compassion, advocacy, and thoughtful support. This is an opportunity to contribute meaningful social work expertise in a care setting where emotional, spiritual, and practical support are central to the patient and family experience.

Responsibilities

• Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist in the development and implementation of goal-directed care plans.
• Provide care under the direction of a physician and participate in IDT conferences, observing, assessing, and sharing information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family.
• Document all patient and family visits, telephone contacts, and referral actions in a timely, accurate, and concise manner in the patient record.
• Conduct reassessments of patient and family needs and provide counseling as required.
• Provide crisis intervention and individual or family counseling when indicated.
• Serve as a care advocate for the patient and family unit.
• Develop and maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate related referrals.
• Provide ongoing counseling related to issues of death and dying to the patient and family as needed.
• Participate as a member of the Bereavement Team as assigned.
• Adhere to all hospice policies.

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• Active LMSW or LLMSW license in Michigan
• Prior experience working with patients and families
• Valid driver's license, car insurance, and reliable transportation
• Eligible to work in the United States
• Experience in home health care or hospice care preferred
