Job summary

AdvisaCare is seeking a compassionate MSW to guide patients and families through the hospice journey with advocacy and psychosocial support. You will collaborate with physicians, care teams, and families to develop and implement individualized care plans in a home-based setting.

Responsibilities include assessments, counseling, care coordination, and bereavement support, with a focus on compassionate, patient-centered care and community resource linkage. A valid Michigan license is required.

Qualifications

• MSW degree from CSWE-accredited program.
• Active LMSW or LLMSW license in Michigan.
• Valid driver's license and reliable transportation.
• Eligible to work in the United States.
• Experience with patients and families in care settings.

Responsibilities

• Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist in goal-directed care plans.
• Provide care under physician direction and participate in IDT conferences, sharing psychosocial, emotional, spiritual, physical, and financial information.
• Document all patient and family visits, calls, and referral actions in the patient record.
• Conduct reassessments of patient and family needs and provide counseling as required.
• Provide crisis intervention and individual or family counseling when indicated.
• Serve as a care advocate for the patient and family unit.
• Develop and maintain contact with community agencies to facilitate referrals.
• Provide ongoing counseling related to issues of death and dying to the patient and family.
• Participate as a member of the Bereavement Team as assigned.
• Adhere to hospice policies.

• Provide crisis intervention and individual or family counseling when indicated.
• Serve as a care advocate for the patient and family unit.
• Develop and maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate related referrals.
• Provide ongoing counseling related to issues of death and dying to the patient and family as needed.
• Participate as a member of the Bereavement Team as assigned.
• Adhere to all hospice policies.

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)
• Active LMSW or LLMSW license in Michigan
• Prior experience working with patients and families
• Valid driver's license, car insurance, and reliable transportation
• Eligible to work in the United States
• Experience in home health care or hospice care preferred
