Job summary

AdvisaCare is hiring a licensed medical social worker to support patients and families in hospice care in Michigan. You will guide families through the hospice journey with empathy and advocacy, offering psychosocial, emotional, and spiritual support and coordinating with the care team.

You will perform assessments, develop care plans, provide counseling, and participate in interdisciplinary team meetings, ensuring compassionate, coordinated care at every stage.

Qualifications

• MSW degree from a CSWE-accredited school is required.
• Active LMSW or LLMSW license in Michigan is required.
• Prior experience with patients and families in hospice/home health.
• Valid driver’s license, car insurance, and reliable transportation.
• Eligible to work in the United States.

Responsibilities

• Perform psychosocial, emotional, spiritual, and bereavement assessments and help develop care plans.
• Provide counseling under physician supervision and participate in IDT conferences.
• Document visits and referrals accurately in the patient record.
• Reassess needs and provide counseling as needed.
• Provide crisis intervention and individual/family counseling.
• Advocate for patients and families and coordinate with community services.
• Maintain contact with community agencies to support referrals.
• Offer ongoing counseling about death and dying.
• Participate in the Bereavement Team as assigned.
• Adhere to hospice policies.

In this role, you will help guide patients and families through the hospice journey with compassion, advocacy, and thoughtful support. Responsibilities

• Perform initial psychosocial, emotional, spiritual, and bereavement assessments and assist in the development and implementation of goal-directed care plans.

• Provide care under the direction of a physician and participate in IDT conferences, observing, assessing, and sharing information regarding psychosocial, emotional, spiritual, physical, and financial conditions affecting the patient and family.

• Document all patient and family visits, telephone contacts, and referral actions in a timely, accurate, and concise manner in the patient record.

• Conduct reassessments of patient and family needs and provide counseling as required.

• Provide crisis intervention and individual or family counseling when indicated.

• Serve as a care advocate for the patient and family unit.

• Develop and maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate related referrals.

• Provide ongoing counseling related to issues of death and dying to the patient and family as needed.

• Participate as a member of the Bereavement Team as assigned.

• Adhere to all hospice policies.

• MSW degree from a school of social work accredited by the Council on Social Work Education (CSWE)

• Active LMSW or LLMSW license in Michigan

• Prior experience working with patients and families

• Valid driver's license, car insurance, and reliable transportation

• Eligible to work in the United States

• Experience in home health care or hospice care preferred
